Collaborative Management Program

The Collaborative Management Program (CMP) was created in 2004 by the passage of HB 04-1451, to promote and encourage local collaboration for the purpose of improved services and better outcomes for the children, youth, and families that we serve.  Douglas County opted into the CMP in 2009, through the implementation of the Crossover Youth Practice Model.

CMP Processes

Through these processes, the CMP can serve children, youth, and their families who are in need of hands-on community resources and case planning, children and youth at risk of truancy, youth who are at risk of or are involved in the juvenile justice system, children and youth who are at risk of or involved with the child welfare system, and professional in need of brainstorming around community resources.

  • Integrated Service and Support Teams (ISST) include two or more system representatives that are present to assist a child/youth/family with developing an integrated service plan directed by family need, identify goals and facilitate collaboration, and participate in the development of the plan.
  • Multi-System Prevention Program is a multisystem prevention program that must include two or more system representatives that establish a program that facilitates collaboration and addresses needs not currently provided within the community.
