Community Health Survey
The first step toward creating a CHA was a Community Health Survey, which gathered input from community members to help identify and understand opinions about health issues and the quality of life in Douglas County. The survey closed on Sept. 7, 2021. View a summary of findings in the Community Health Assessment Update.
The information collected during phase one – the Community Health Survey – was compiled by HMA, who then conducted meetings across the County virtually and in-person. HMA shared the results from the survey and gathered additional input about county residents’ health priorities. The meetings were held in December 2021.
HMA used information learned from the survey and the community meetings to create the CHA, which they presented to the County. The priorities identified will be used to develop the Douglas County Public Health Improvement Plan (PHIP). The health improvement plan will aid the county in program planning and budgeting resources over the next few years.
Douglas County contracted with Health Management Associates, Inc. (HMA), a public health and health care research and consulting company to create a Public Health Improvement Plan (PHIP) specific to Douglas County and the public health needs identified in the CHA.
