Applicants must meet the following eligibility requirements for DC Cares:
- Income of less than 125% of the Federal Poverty Level (approx. $30,000 for a family of 4)
- Reside legally in the United States
- Valid Colorado Driver’s License or Identification
- Resident of Douglas County (for homeless residents the following factors may be considered when determining residency: child enrolled in Douglas County School District school, employment in Douglas County, benefits and services obtained through Douglas County Human Services or documents proof of last permanent residence in Douglas County)
Residents interested in DC Cares should complete the online inquiry or contact one of the agencies listed below. After submission of the inquiry form, residents who meet initial screening standards will complete an intake assessment and screening with one of our partner agencies. This process can take up to six weeks. All adult members of the household are expected to participate in the application and assessment process.
Framework of Care – DC Cares utilizes a WrapAround framework to provide care and support to families and individuals enrolled in the program. Facilitators work diligently with each person participating in DC Cares to provide a caring environment that focuses on creating hope, building trust, identifying motivation, causing activation, continual repetition and achieving transformation.
Framework of Support – DC Cares recognizes that transformation is best achieved by participants when certain conditions are in place. A framework of support has been created which helps ensure the stability of the family, increases the capacity of each individual and builds the capability of the participants. This allows the family to capitalize on the opportunities they encounter to become self-sufficient. A unique set of tools, resources, and services are provided to support each family while working to achieve their identified goals.
